There are at least two ways to accomplish this feat over and above being a genius at balance. One way allows you to cause the balanced coin to spin after being balanced. This Tango magic version does not permit spinning but you can take your finger and cause the balanced coin to rotate. The effect is fairly easy to do and as with all magic, if done at the right time in the right way is baffling. With just a bit of slight of hand you can hand two coins out to be examined. The 3 star rating is because after achieving balance, being able to cause the coin to spin is a second applause-getter while forcing it around to another angle with the forefinger is not quite as exciting. Good impromptu or bar trick - not really a stage effect because of the size of the props.
